At Tulsa Retina Consultants, we desire to provide the highest level of care to our patients. Upon the arrival of your appointment date, you will be greeted by our receptionist and asked to fill out appropriate forms, including a detailed medical history and a full list of current medications. This information is extremely important for our physician and is important for the interpretation of retinal findings. You can access these forms below.

Once you have completed the necessary paperwork, you will begin the evaluation process. All patients are dilated at each visit to allow for full evaluation of the retina. Dilation often lasts up to 8 hours, so please be prepared for this event. We suggest having a driver come with you to your examination to assist you once you leave our facility. Once the initial workup has been completed (including vision evaluation, eye pressure check, and initial imaging), you will be allowed to dilate fully before the retinal examination. Often the physician will order further testing once the retinal evaluation has been completed. Due to the extent of the initial evaluation, please be prepared for this exam to take a couple of hours to complete. We appreciate your understanding and patience through this process.
